Abstract

Data inconsistency in Standard Work Element Sheet (SWES) documentation can lead to material mismatches, incorrect specifications, and production delays in heavy equipment assembly processes. This study aims to identify the dominant root causes of SWES data inconsistency and develop improvement mechanisms to enhance data update accuracy at PT XYZ. A qualitative case study approach was applied through observation, interviews, and document analysis, supported by Fishbone Diagram (6M), Pareto Analysis, and 5 Whys. The results identified RC-1 (human error in Textbox input) and RC-4 (dual-source data without synchronization) as the dominant causes, contributing 27.6% and 23.7% of 152 inconsistency cases, respectively. Before implementation, no automated mechanism existed to detect mismatches, and manual dual-entry created repetitive data handling. After implementing a VBA-based alignment detection mechanism, auto-synchronization, and a standardized workflow, all 10 compatible SWES sequences were successfully synchronized and achieved fully aligned outputs, reducing inconsistency risk and improving data update reliability.
